About Prudence Chan – Consultant Occupational Therapist & Dementia Care Specialist

Prudence Chan is a Consultant Occupational Therapist and the founder of Pacific Rehab Group.

Prudence possesses 18 years of clinical experiences in supporting individuals and seniors with cognitive impairment & dementia of all subtypes in the community of Singapore.

Formerly an Occupational Therapist of Changi Hospitals ( CGH ) , Tan Tock Seng Hospital ( Community Rehabilitation Programme CRP ) and subsequently, a Senior Occupational Therapist of Ren Ci Hospital, she is highly specialized in the management of dementia clients as well as complex behavioral cases arisen from various dementia subtypes.

She is presently the Visiting Consultant Dementia Care Specialist of Lions Home for the Elders ( LHE ) – leading the digitization processes and SMART ward and the adoption of tech and innovation to support and elevate the quality of life of persons with dementia.

As an experienced Dementia Care Practitioner, Prudence possess a deep understanding in support for the following dementia subtypes:

- Mild Cognitive Impairment ( MCI )

- Alzheimer’s Disease ( AD )

- Vascular Dementia

- Frontal Temporal Lobe Dementia

- Dementia with Lewy Bodies

She is also the Subject Matter Expert ( SME ) of Dementia for National University of Singapore ( NUS ) for School of Integrative Sciences – in Content Creation for Innovative Projects ( iPADs ) in collaboration with Villa Francis Home for the Aged.

Prudence bridges clinical expertise & specializes in creating personalized, practical care plans that focus on non-pharmacological interventions, effective communication techniques, and creating a safe, dementia-friendly environment.

Her mission is to be a trusted guide and advocate, empowering families with the knowledge and support they need to transform challenges into moments of connection and grace.
