My name is Prudence CHAN
Prudence Chan ( Ms ) is a Consultant Occupational Therapist, Dementia Care Specialist and the Clinical Director of Pacific Rehab Group.
She holds Bachelor in Occupational Therapy ( La Trobe University ) in 2008 & Diploma i%n Occupational Therapy ( Singapore ) in 2006. In 2017, she started her Dementia Care specialty training in 2017 and received Master’s Degree in Dementia Studies University of Stirling ( Scotland ).
At the start of her clinical work, she has been practicing in Changi General Hospital, Gleneagles Hospital, Tan Tock Seng Hospital’s Community Rehab Programme ( CRP ), Ren Ci Community Hospital and Singapore Cancer Society, specializing in Geriatric Medine, Neurological Rehabilitation and Dementia Care Practice.
As she progressed into specialization into dementia care, her dementia care work has evolved into supporting clinical care, protocols, assessments, consultation and personalized treatment planning for patients in the sector, from Yong-En Dementia Day Care, Apex Harmony Lodge , Lions Home for the Elders & Villa Francis Home for the Aged.

Academically, she has been involved as a Subject Matter Expert of Dementia Care Specialist, providing consultancy work to the Graduate School of Integrative Sciences & Engineering for National University of Singapore ( NUS ) and rendering advise on the appropriate structuring of tablet-based activities for their research topic on using technology in support for persons with dementia.
Prudence is a member of Alzheimer’s Scotland.
Prudence is also a Board Member of National Arthritis Foundation ( NAF ) as part of her involvement in the community level.
She is passionate about geriatric care, in particular dementia care. With Dementia Care Consulting, she hopes to bring a state of health and well being to older persons as much as possible, and to work with their families and corporate organizations involved in senior care to improve the quality of life for older persons and their caregivers.
The scope of dementia care spans awareness talks, workshops & consulting work to empower organizations to ramp up equipping their care staff to care competency and improved care to persons with dementia through evidence-based approaches.

For the month of May, in collaboration with Lions Home for the Elders, Pacific Rehab Group has successfully undertaken to conceptualize and implement the adoption of Doll Therapy Project for More than 50 Dementia Residents of Lions Home for the Elders. The project was well-received with nurses reporting residents with dementia showing lesser episodes of distress and improved mood amongst the residents.
